Here at Revived, the goal is to provide ample support for clients. Appointments can be set based on your needs and will be discussed in more detail during your initial consult.
To get the most out of your session, be sure to complete all necessary paperwork sent to you at least 24 hours prior to your initial appointment (this includes your medical history, food journal, medications/supplements, my symptoms questionnaire etc.). Be ready to discuss your health goals, challenges, and any specific questions you have.
Revived Nutrition Counseling is currently unable to accept insurance at this time due to limitations with Michigan state licensure. HSA/FSA payments are accepted and a superbill (medical receipt used for reimbursement of services) can be provided upon request.
A referral is not necessary to receive nutrition counseling. However, it is needed if you plan to submit a superbill to your insurance.

Due to state licensure laws, clients must reside in the state of Michigan. Clients outside of Michigan should connect with me and may be accepted on a case by case basis pending the state they reside in.
An RD has completed extensive education, training, and passed a national exam to earn their credential. They are required to keep up with this credential by completing 75 continuing education credits every five years. They are qualified to provide evidence-based nutrition advice and medical nutrition therapy. The term “nutritionist” is less regulated and does not require a formal certification or credentialling process.
